Your complaint must include the following information:

A detailed description of the allegedly infringing content, including its specific location on the website or application, providing sufficient detail to assist Kaizikd in locating the content;
If a design patent is involved, clearly identify the design you believe to be infringed and provide the registration number;
If a utility model patent is involved, clearly identify the utility model you claim is infringed and provide a court order or judgment supporting your claim;
Your full company name, organization (if applicable), mailing address, telephone number, and email address;
A statement that you believe in good faith that the use of the allegedly infringing content has not been authorized by the intellectual property owner, its agent, or by law;
A statement based on a perjury penalty, confirming that the information in the notice is accurate and that you are the intellectual property owner or authorized to act on behalf of the intellectual property owner.

Upon receiving such notification, we reserve the right to take the following measures: remove or block access to the allegedly infringing content; disconnect all links pointing to the content; notify the alleged infringer that access to the relevant content has been removed or blocked; terminate access to and use of Kaizikd services by users or third parties who repeatedly infringe on the intellectual property rights of Kaizikd or others, or handle the matter in accordance with other provisions of the Terms of Use or Policies.
